About the Role Position Type: Per Diem Setting: Skilled Nursing Facility (Long-Term Care) Key Responsibilities Evaluate and treat patients with functional, cognitive, and ADL impairments. Create and implement individualized care plans. Collaborate with interdisciplinary team to ensure quality outcomes. Maintain accurate documentation and regulatory compliance. Obtain and update required physician orders. Qualifications Degree in Occupational Therapy (BS, MS, MA, or OTD) from an AOTA-accredited program. Current license as an Occupational Therapist in the state of practice. Certified by NBCOT as an OTR if required by the state. Strong communication and organizational skills.
