Overview Position: Qualified Intellectual Disabilities Professional (QIDP) – Community Resources and Supports (CRS). Location: 1802 W Berteau, Chicago, IL. Shift: Monday – Friday, 8:00 a.m. – 4:00 p.m. (hours may vary). A flexible schedule is required and on‑call availability is needed. Anixter Center is a community‑focused 501(c)(3) organization with a $17 million budget, 22 locations and 200 staff. Since 1919 we have served people with disabilities, behavioral health challenges, and Deaf/DeafBlind/Hard of Hearing individuals. Salary: $53,000.00 – $56,000.00 per year. Responsibilities Lead QIDP case management for the Community Resources and Supports (CRS) and Community Day Services (CDS) Legacy and Choice programs. Maintain caseloads and update monitoring spreadsheets to ensure compliance with Rule 119 and submission of BQM, BALC surveys and audits. Integrate, coordinate and monitor active schedules, strategies and outcomes for individuals with disabilities. Supervise Direct Support Professionals (DSP) and provide training on client strategies and medical/behavioral needs. Complete case‑management and medical service paperwork within DHS standards and timeframes to ensure appropriate rates. Monitor and assess awarded hours, collaborate with the Primary QIDP, Program Managers and Director to apply for additional support funding and respond to DHS documentation requests. Maintain program compliance with funding body requirements and transfer monthly billing for CRS programs in collaboration with Program Managers. Perform quality‑assurance duties, including knowledge of DHS Rule 119, client records, and medical documentation. Participate in required training, meetings and committees as directed. Collaborate with the BCBA, Special Support Team (SST) and other team members to improve services and promote continuity. Serve as an active member of the Community Support Team (CST) for clients receiving services across CRS programs. Ensure timely completion of all client paperwork and documentation, signed annually and current per Agency and DHS policy. Coordinate intake, admission, transfers and discharge with Outreach Manager, Program Manager and Primary QIDP. Collaborate with ISC and Home‑Based Facilitator or Service Direct Administrator to complete required annual assessments and updates. Provide direct support during problem or crisis situations as required. Assist accounting in following up on rejected billing. Develop cooperative relationships with residential facilities, families and community agencies to facilitate referrals and improve program services. Respond to medical and behavior crises in accordance with agency policy, including related paperwork. Ensure accurate and timely completion of documentation within Anixter Center’s Electronic Health Record (EHR). Confirm daily and weekly data tracking for each client’s outcomes and behavior. Complete required On‑The‑Job training (OJT) and new DSP training within the 120‑day period per DHS. Maintain relationships with CST members. Stay updated on program changes and communicate effectively internally and externally. Perform quality‑assurance duties on files, maintaining knowledge of DHS rules and regulations. Maintain program records, schedules and reports. Provide coverage during absences, vacancies or emergencies, including driving agency vehicles and transporting clients. Adhere to mandated reporter, OIG and DHS guidelines. Complete incident reports and ensure timely follow‑up within a 4‑hour timeframe; report critical incidents via CIRAS. Communicate incidents and updates to CST, follow‑ups and schedules. Maintain professional communication with co‑workers, clients’ parents/guardians, employers and stakeholders. Complete required on‑site and off‑site training and certificates (e.g., Department of Human Services, crisis intervention, etc.). Maintain confidentiality of records per agency policies. Perform other related duties as assigned. Qualifications Bachelor’s degree in social service or related field; at least two years of management experience. Qualified Intellectual Disabilities Professional (QIDP) certification or ability to obtain certification within six months of hire. Maintenance of CEUs for QIDP certification throughout employment. One year of experience working with people with developmental disabilities. Demonstrated ability to work effectively with diverse populations. Collaboration skills with professionals and diverse personalities (clients, parents, staff, employers, funding representatives). Outstanding interpersonal, employee coaching and development skills. Ability to multi‑task in high‑stress, fast‑paced environments. Excellent interpersonal, organizational, verbal and written communication skills. Basic computer literacy; navigation of internet, training systems, and recall of login credentials. Proficiency in American Sign Language (ASL) preferred but not required. Ability to set priorities and multi‑task in high stress situations. Commitment to company values and a diverse, inclusive, equitable work environment. Valid driver’s license, meeting agency insurance requirements and capable of driving larger vehicles (12‑15 passenger van, agency bus, etc.). Work Environment Physical demands: standing, walking, sitting, use of hands, reaching, climbing stairs, balancing, stooping, kneeling, crouching, crawling, speaking, hearing, tasting and smelling. Occasionally lift and/or move up to 50 pounds. Vision requirements: close, distance, color, peripheral, depth perception, and focus adjustment. Work environment: exposure to weather conditions; moderate noise level.
