Job Title: Care Manager Profession: RN Specialty: Case Management Duration: 13 weeks Shift: Days Hours per Shift: 40 Experience: Two (2) years of health care experience as a Registered Nurse License: Licensed to practice as a Registered Nurse in the state of North Carolina Certifications: Nursing Diploma or ADN/ASN from an accredited school of Nursing Must-Have: DISCHARGE PLANNING CRITICAL THINKING CASE MANAGEMENT UTILIZATION REVIEW PATIENT ASSESSMENT Description: The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction. It promotes continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning. The Care Manager must be a highly organized professional with great attention to detail. They should be adaptable to frequent change and compliant with regulatory and departmental guidelines and policies. Essential Duties include identifying cases and prioritizing daily work lists to review patients and identify new admissions. Conduct and document assessments and a plan of care per departmental guidelines. Participate in daily care management touchpoints as established. Consult with social workers when necessary based on established criteria. Communicate with the Care Management Assistant to share priorities when indicated. Attend and actively participate in meetings for assigned units to provide and receive information on patient progression. Alert the care team to concerns that could impact the anticipated discharge of the patient. Modify discharge plans based on information shared in meetings. Assist in identifying the expected discharge date. Complete follow-up from meetings as appropriate. Meet with the Utilization Manager and Social Worker after meetings as necessary. Attend weekly complex care meetings and present on patients during these discussions. Collaborate to problem-solve issues with complex patients and identify trends. Formulate potential solutions with the Utilization Manager and Social Worker while continuously monitoring cases. Proactively identify high-risk cases that need to be escalated. Complete follow-up after meetings as assigned. Discuss barriers to discharge and psychosocial concerns with appropriate members of the multidisciplinary team. Coordinate family meetings as necessary to support the progression of care. Provide education on community resources, support groups, and appropriate resources to patients, families, and the care team. Educate and coordinate referrals to community resources and post-acute providers as necessary. Communicate medical milestones for transition with the patient and family. Identify patients with barriers to discharge based on experiences and meetings. Monitor observation patients throughout the day to ensure appropriate progression of care. Assess the discharge plan to determine needs post-discharge and communicate these needs. Identify required authorization for post-discharge services and refer to the appropriate service provider. Participate in medication resource management for patients when needed. Verify the patient's understanding and agreement of the discharge plan. Refer administrative tasks to the Care Management Assistant. Consult with Social Worker and/or Utilization Manager as per departmental protocol.
