Case Coordinator

Position Specific Information Limited grant funded position through September 2027 as part of the EASE program. Enhancing wellness through affirming services and education (EASE) is a program providing substance use and mental health treatment for clients in the LGBTQIA+ community, MSM (Men having Sex with Men) population, HIV individuals and those individuals at high‑risk for HIV. The selected candidate will provide case management services and link clients to community resources and work with a team to assist clients in achieving mental, physical, and emotional mental health and wellness. Monday – Friday, 8 AM to 5 PM, with flexibility as needed to include occasional weekends. Full time in office / on‑site position located on Billingsley Road in Charlotte, NC 28211. Minimum Qualifications Experience: Minimum of two years in an applied setting such as substance use, mental health, developmental disabilities or other health conditions. Education: Bachelor's degree in a human services related field. Combination of relevant education and relevant experience accepted: Yes. Licenses and Certifications Requires a valid North Carolina or South Carolina Driver's License. Requires County Driving Privileges. Annual Motor Vehicle Record (MVR) check required. Preferred Qualifications Experience performing case management duties with at risk populations. Previous experience working with individuals 18–35 years of age, who identify with the LGBTQIA+ community and may be experiencing mental health and/or substance use issues. #J-18808-Ljbffr
