Home Health Physical Therapist

Assignment Duration

Schedule

Day schedule: 5x8 hours (08:00 - 17:00)

Job Description

We are seeking a skilled Home Health Physical Therapist to provide high-quality patient care in a home health setting. The ideal candidate will be responsible for conducting patient assessments, developing treatment plans, and providing engaging therapy interventions to support patient rehabilitation.

Responsibilities

• Perform comprehensive assessments of patients' physical conditions and functional capabilities.
• Develop and implement individualized treatment plans based on patient needs.
• Utilize equipment and devices such as gait belts, rolling walkers, and sit-to-stand lifts.
• Educate patients and families on home exercise programs and safety precautions.
• Monitor patient progress and adjust treatment plans as necessary.
• Document patient care and progress accurately in compliance with regulations.
• Conduct re-assessments and ensure proper billing practices.

Required Experience / Certifications / Licensure

• Minimum 1 year of home health experience required.
• State licensure as a Physical Therapist is mandatory.
• BLS certification required.
• OASIS training preferred.
• Valid driver's license and reliable transportation for home visits.
• COVID vaccination or approved exemption and flu shot required.
