Confidential Opportunity – Social Services Director (Long-Term Care)

A skilled nursing and rehabilitation facility is seeking an experienced Social Services Director to lead the Social Services department and support the psychosocial well-being of residents and their families.

Position Summary

The Social Services Director oversees all aspects of resident social services, including admissions support, care planning, discharge planning, resident advocacy, and compliance with federal and state regulations. This leadership role partners closely with the interdisciplinary team to enhance resident outcomes and quality of life.

Key Responsibilities

• Direct and manage the Social Services department

• Conduct and oversee psychosocial assessments and care plans

• Coordinate discharge planning and community resource referrals

• Support residents and families through transitions and ongoing care needs

• Participate in interdisciplinary team meetings and care conferences

• Advocate for resident rights and address concerns or grievances

• Ensure compliance with all regulatory requirements and documentation standards

• Mentor and support Social Services staff

Qualifications

• Bachelor's degree in Social Work, Human Services, or related field required

• Long-term care or skilled nursing experience preferred

• Previous leadership or supervisory experience strongly preferred

• Knowledge of Medicare, Medicaid, discharge planning, and long-term care regulations

• Excellent communication, organizational, and interpersonal skills
