SKILLS REQUIRED
Primary (Must-Have):
• Active Alabama RN License or Compact License required
• AHA or Red Cross BLS Certification required
• Minimum 2 years of RN experience required
• Experience in utilization review and/or case management preferred
• Must be willing to float to other units and hospitals as needed
Secondary (Good to Have):
• BSN or related bachelor’s degree in a healthcare field preferred
• Strong discharge planning and care coordination experience
• Excellent interdisciplinary communication and collaboration skills
• Knowledge of healthcare resource utilization and patient care planning
POSITION OVERVIEW
We are seeking an experienced RN Care Coordinator for a 13-week travel assignment in Alabama. This role is responsible for coordinating patient care throughout the acute care episode while collaborating with interdisciplinary healthcare teams to support safe discharge planning and positive clinical outcomes. The ideal candidate will have strong case management experience, excellent communication skills, and the ability to work collaboratively across the continuum of care.
ROLES & RESPONSIBILITIES
• Coordinate patient care plans and discharge planning activities across the continuum of care
• Collaborate with physicians, nurses, and interdisciplinary teams to ensure effective patient outcomes
• Monitor healthcare resource utilization and support quality patient care initiatives
• Facilitate communication between internal and external healthcare providers and patient support systems
• Maintain accurate patient documentation and ensure compliance with healthcare regulations and organizational policies
