Diabetes Care and Education Specialist

Job Summary: Assesses learning needs, develops an education plan, provides education and evaluates outcomes with patients and caregivers with diabetes. Provides education in inpatient and/or outpatient settings, and participates on teams for policy and protocol development. Responsibilities Assess patient/family/caregiver baseline knowledge about diabetes, diabetes management, and complications, and readiness to learn. Provide diabetes and self‑management education, including technical training in capillary blood glucose measurement, safe injection technique, and insulin pump management. Monitor and evaluate patient responses to therapeutic interventions and collaborate with the health‑care team to adjust the plan of care as needed. Coordinate with staff to integrate medical therapy into overall diabetes management; consult nursing and physician staff about diabetes issues and provide just‑in‑time education. Utilize health‑literacy principles to deliver effective education to patients, families, caregivers, and health‑care team members. Participate in continuous improvement by monitoring hypoglycemia/hyperglycemia trends and using evidence to improve diabetes‑care delivery structures and processes. Oversee planning, implementation, and evaluation of DSMES program(s) as required. Arrange and coordinate activities of the Advisory Group. Serve as liaison between DSMES staff, the Advisory Group, other departments and administration. Monitor and facilitate maintenance of DSMES staff qualifications (CE credits, licensures, registrations). Ensure ADA Recognition and participate in evaluation of program effectiveness. Perform other duties as assigned. Qualifications Graduate of an accredited school of nursing, pharmacy, or registered dietitian program. No experience required. Licenses and Certifications Basic Life Support Healthcare Provider (BLS HCP) required within 90 days of employment. Other resuscitation certifications may be required depending on department. State‑specific licensure: Illinois – BLS HCP (AHA), RN (IDFPR) or Pharmacist (IDFPR) or RD (CDR) or CDE (NCBDE). Missouri – BLS HCP (AHA) and Pharmacist (MDPR) or RN (Compact or MDPR) or RD (CDR) or CDE (NCBDE). Oklahoma – BLS HCP (AHA) and Pharmacist (OKSPB) or RN (Compact or OKBN) or RD (CDR) or CDE (NCBDE). Wisconsin – BLS HCP (AHA) and Pharmacist (WSPS) or RN (Compact or WSPS) or RD (CDR) or CDE (NCBDE). Work Shift Day Shift (United States of America).
