Locum - Physician - Infectious Disease Prescott, AZ

Job Description

Schedule/Availability Requirements:

July 29-August 25, covering for a provider on vacation

site will not consider: 1 week per month or 1 week on 1 week off, must be able to provide 2 weeks per month minimum

Setting: inpatient

Duties: inpatient consultations: and IVIg orders for the Infusion Center

Days per month: Full time Mon-Fri

Required Procedures: in office dressing changes/wound care and IVIg orders for the Infusion Center.

EMR: CERNER

Support Staff: 1 MA, 1 front office

Reason for Coverage: Long term provider is retired

Deal breakers: need someone who can come for an extended time, not just 1-2 weeks.

Credentialing Information:

Timeframe: 30-90 days, will issue emergency privileges for providers who can start asap
Requirements: clean malp and background for rushed credentialing

Additional Details / Questions Answered by Facility:

7-12 patients per day, provider reviews all incoming referrals to approve
First day info- orientation to the clinic space and EMR training, on first day of patients Cerner support staff is in clinic to answer any questions

Submission Requirements

Board Certification - REQUIRED (can accept Board Eligible candidates)
AZ License or IMLC (LOQ in hand) - REQUIRED
Residency/Fellowship - REQUIRED
DEA - REQUIRED
Clean Malpractice - REQUIRED for rushed credentialing
Availability at time of name clear - REQUIRED
