Home-Based Primary Care NP — Flexible Hours, Low Call

HarmonyCares seeks a Nurse Practitioner to provide in-home primary care to high-acuity patients in the Troy, MI area. The clinician will assess, diagnose, and manage patient care in the home setting, coordinating with a multidisciplinary team to address complex medical needs. The ideal candidate will hold Michigan NP licensure, a current DEA license, and board certification.
