Responsibilities
- Conduct initial physical therapy evaluations and ongoing reassessments for residents
- Develop, implement, and monitor individualized treatment plans to improve mobility and functional independence
- Provide interventions such as therapeutic exercises, gait training, balance training, and functional activities
- Monitor resident progress, adjust plans as needed, and document outcomes for charting and billing
- Collaborate with an interdisciplinary care team and educate residents and families on care plans and safety
- Ensure patient safety during therapy and maintain compliance with facility policies

Requirements
- Active physical therapy license with eligibility to practice in New Mexico
- Ability to work effectively in a skilled nursing facility setting
- Strong clinical assessment, documentation, and communication skills
- Willingness to participate in a 13-week travel assignment in Albuquerque, NM
- Experience documenting outcomes and coordinating care with interdisciplinary teams (preferred)
