Wellness Coordinator

Blakeford Community Support SpecialistESSENTIAL FUNCTIONS & RESPONSIBILITIES: Serve as the primary relationship manager for program members and their families or designated contacts Conduct regular health, wellness, and memory screenings and assessments Coordinate and oversee member care and services Collect, track, and analyze member health and wellness data and maintain accurate documentation Coordinate wellness activities, community resources, vendors, and interventions Collaborate across all service lines and support program growth and outreach Other duties as assigned EDUCATION & EXPERIENCE: Bachelor's degree in nursing, social work, occupational therapy, exercise physiology, or other wellness or rehabilitation disciplines 5+ years experience working with older adults in geriatrics, wellness, rehabilitation, care coordination or community-based services Why You'll Love Being a Blakeford Community Support Specialist: Competitive pay Health, dental, vision insurance and other benefits Company-paid life and short-term disability On-site team member gym Paid time off with cashout option 403(b) with employer match Tuition reimbursement and scholarship programs Award-winning team member appreciation events and so much more!
