Job Title: Travel Physical Therapist (PT) – Home Health

Location: Longview, WA (Coverage within a 50‑mile radius)

Employment Type: Full‑Time, Travel Assignment

Weekly Hour Guarantee: 40 hours

Shift Schedule: Day shift, Monday–Friday, 8:00 AM – 4:30 PM

On‑Call / Weekend Requirement: None

Mileage Reimbursement: Yes (between patient visits, first patient to last patient)

Job Summary

We are seeking an experienced and motivated Travel Physical Therapist (PT) to deliver high‑quality home health physical therapy services within a 50‑mile service area. This role focuses on comprehensive evaluations, skilled treatment, and care coordination throughout the entire episode of care. The ideal candidate brings strong clinical expertise, home health experience, and the ability to work independently in a patient‑centered home environment.

Key Responsibilities

Perform Start of Care (SOC) visits, physical therapy evaluations, Resumption of Care (ROC) visits, recertifications, and subsequent therapy visits.

Assess patient functional status, mobility, strength, balance, and safety within the home environment.

Develop, implement, and revise individualized physical therapy plans of care in collaboration with physicians and the interdisciplinary care team.

Provide skilled physical therapy interventions including therapeutic exercise, gait training, balance training, mobility training, and pain management.

Monitor patient progress, measure outcomes, and adjust treatment plans to meet established goals.

Educate patients and caregivers on treatment plans, home exercise programs, fall prevention, and safety strategies.

Complete accurate, timely, and compliant documentation in the electronic medical record (Homecare Homebase).

Communicate effectively with physicians, nurses, and other care team members to ensure coordinated and continuous care.

Maintain compliance with all agency policies, regulatory standards, and quality requirements.

Travel daily between patient homes within the assigned coverage area.

Required Qualifications

Active Physical Therapist license and/or Doctor of Physical Therapy (DPT) credential.

Current CPR certification.

Valid driver’s license with reliable transportation.

Ability to work independently in a field‑based home health role.

Preferred Skills & Experience

Prior home health physical therapy experience.

Previous use of Homecare Homebase (HCHB) EMR.

Strong clinical assessment and documentation skills.
