Hospice Social Worker
Compassus
Mercy Health Home Care and Hospice by Compassus is seeking a Hospice Social Worker to provide psychosocial assessments, counseling, and support within the interdisciplinary team. You will promote Compassus values and coordinate care planning across settings to optimize coping for patients and families. The role emphasizes advance care planning, living will/POA facilitation, and linkage to community resources, while upholding ethical conduct and nondiscrimination as part of compassionate hospice #J-18808-Ljbffr
