THIS IS A HYBRID ROLE. IT IS REQUIRED TO RESIDE WITHIN ONE OF THE LISTED LOCATIONS FOR CONSIDERATION.

Monday through Friday
8:00 AM - 5:00 PM EST
Pay Rate $34.00
50%-75% Field Visits (mileage reimbursement)

City of Homestead, FL within Miami-Dade County
Reside in the City of Cutler Bay (33157, 33197, 33177, 33189,33187, 33157, 33176, and 33186 zip code specifically)
Reside in Miami-Dade County, FL (West Hialeah) (33010, 33012, 33013, 33014, 33015, 33016, 33018, and 33027 specifically).
Reside in Hialeah (Must be in 33010, 33012, 33013, 33014, 33015, 33016, 33018)
Reside in Miami-Dade County, FL (MUST BE IN ONE OF THESE ZIP CODES. 33192, 33182, 33172, 33193).
Reside in North Miami, Aventura, Miramar, Doral, or Coral Gables.
MUST be located in Miami-Dade County in zip codes (33030, 33033, 33035, 33031, 33032)
MUST be located in Miami-Dade County in one of these zip codes (33126, 33125, 33015, 33018, 33133)

Summary:

Healthcare Consultant is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Healthcare Consultant will effectively manage a caseload that includes supportive and medically complex members. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration.

Requirements:

Bachelor's degree in Social Work, Health Science, Healthcare Administration or a related field
At least 3 years of case management experience
At least 1 year of long-term care experience
Bilingual Spanish/English (fluent)
Proficiency in Microsoft Office such as Word, Outlook, Excel, and Teams
Will travel approximately 50%-75% of the time within the region, seeing Members at home, in assisted living facilities, and nursing homes to complete assessments
Reliable transportation and insurance
