CHRISTUS Health Rehabilitation Hospital in Longview, TX seeks a Licensed Case Manager to join our inpatient rehab team. You will coordinate interdisciplinary care, manage discharge planning, and communicate patient progress to families and the care team. Licensure as RN, Social Worker, Respiratory Therapist, PT, OT, or SLP is required; CCM/ACM/RN-BC/ARN certifications are preferred.
