Job Overview

The HOPES Home Visitor is responsible for addressing the social service needs of participants in the AVANCE Dallas HOPES program and is responsible for providing educational support to all families through their home visits and maintaining an active caseload of families in greatest need. The HOPES Home Visitor is responsible for always creating a welcoming atmosphere for AVANCE internal employees and AVANCE external program participants.

Job Responsibilities
  • Program Fidelity:
  • Recruitment of families to participate in the AVANCE HOPES program based on the program requirements.
  • Conduct bi-weekly 30-45-minute home visits with each family.
  • Schedule and plans home visits with the participation of only the parents or legal guardian.
  • Create and maintain respectful partnerships with families.
  • Drive to the family home and deliver health, educational, nutritional, and child growth and development information to parents via bi-weekly meetings.
  • Ensures home visits are conducted based on program requirements.
  • Maintains an assigned caseload of families.
  • Complete family developmental screenings and all other required assessments with families in a timely manner.
  • Works with parents to strengthen the family's knowledge of child development, including assisting parents to understand how children grow and learn, and planning and conducting child education activities with the parents which meet the child's intellectual, physical, emotional, and social needs.
  • Assists parents in strengthening the families' knowledge of health and nutrition, including integrating health and nutrition education into the program, coordinating with other staff and parents regarding health screenings for family members, and providing information and referrals, if necessary.
  • Provide support in parent and child activities that encourage developmentally appropriate activities for the changing needs of infants and toddlers.
  • Curriculum Coordination:
  • Meets regularly and maintains active communication with other program staff.
  • Develops and maintains community resource information and referral procedures.
  • Assists parents to strengthen their knowledge of community resources and support parents in problem-solving.
  • Assist and support families in locating resources and scheduling appointments with community resources that best meet their needs.
  • Provides support to other professional staff members involved in family interventions.
  • Refers children with language delays, developmental delays, and physical /medical problems to appropriate community services.

Requirements

Education

Required:
  • Bachelor's Degree in Social Work or related field
Preferred:
  • N/A
Work Experience

Required:
  • Minimum of one (1) year experience as a case worker, or other relevant experience with a social service agency
Preferred:
  • Minimum of two (2) years of diversified experience securing social service and community resources for clients in the Dallas area
Additional Skills
  • Must be able to speak, read, and write fluently in Spanish and English
  • Strong communication, organizational and planning skills.
  • Ability to effectively intervene with clients and client groups
Travel
  • Ability to travel to various sites: Dependable transportation is required daily with a valid driver's license, a clean driving record (subject to annual MVR (Motor Vehicle Report) check and policy), current auto liability insurance and be bondable by AVANCE, Inc.
  • Travel required up to 50% of the time for work-related site visits, meetings, and functions.
Working Conditions
  • Work is generally performed in an office environment.
  • Noise level in the work environment is moderate to occasionally loud (examples: business office with computers and printers, light to moderate traffic, human voices).
  • Standard office equipment generally used includes:
  • Telephone
  • Personal Computer (monitor, keyboard, and mouse) or Tablet
  • Printer/Photocopy Machine
  • Calculator
  • May be required to operate a motor vehicle during the course of duties.

Mental and Physical Demands

Maintain emotional control under stress; work with interruptions and deadlines; walking, standing, stooping, bending, pulling and pushing, use hands to finger, handle or feel; reach with hands and arms; occasional: lifting/carrying, pulling/pushing 10-30 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Environmental Factors

Occasional exposure to: biological hazards (communicable diseases, bacteria, insects, mold, fungi, etc.). Occasional expose to weather related conditions when working in outside environment/neighborhoods and events.
