Schedule: Mon-Fri 8:30am - 5:00pm

JOB SUMMARY: Under the supervision of the Manager of Case Management, the Case Manager will provide a coordinated and customized care plan for at-risk elderly seniors (60 and over) within the Orange Countyservice area. The Case Manager will be responsible for conducting comprehensive assessments andconnecting clients to services that promote independence and overall quality of life. The case load for a full-time Case Manager will be determined based on experience and client complexity.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Conducts client-specific assessment of needs, identifies problems, and establishes client-centered

immediate needs and long-term goals.

2. Reassesses ongoing client needs and eligibility every three months and adjusts services as

appropriate.

3. Coordinates a network of supportive services and entitlements (formal and informal) and provides

information and referrals consistent with mutually developed care plans.

4. Acts as an advocate for the client and coordinates with vendors to ensure satisfaction with services

provided.

5. Coordinates and monitors homemaking, personal care, and chore services through contracted

vendors.

6. Maintains required records and reports in compliance with department, agency, local, state, and

federal requirements.

7. Participates in agency meetings, team meetings, and training.

8.Assists in maintaining a current directory of community services containing older adult human service

programs and agencies in Orange County.

9. Conducts in-service training with volunteers on case management.

10. Serves as a representative of the organization and primary point of contact.

11. Represents the agency at county and agency meetings at the discretion of the Department Manager.

12. Participates in community outreach to inform and educate the general community about the home-

delivered meals program as well as other agency programs.

13. Performs other duties as assigned relevant to Department functions.

14. Utilizes personal transportation to drive to client homes, partner agencies, and community sites.

EDUCATION, EXPERIENCE AND QUALIFICATIONS:

• Bachelor’s degree in human services, Sociology, Psychology, social work or other related social

services field of study.

• Prior work experience with elderly people is desired.

• Sensitivity to and awareness of the special needs and resources for this population.

• Keen assessment and evaluation skills with a thorough knowledge of case management principles

and techniques.

• Ability to maintain professional and confidential standards in client business-related activities.

• Must be able to communicate clearly and effectively in person, over the phone, and via written

communication.

• Ability to develop cooperative working relationships with community resources, clients, informal

support sources, and other employees.

• Working knowledge of programs and services available in Orange County for seniors.

• Must pass post-offer background check.

• Bilingual preferred – Spanish or Vietnamese

PHYSICAL JOB REQUIREMENTS:

• Ability to sit, stand, and walk for extended periods throughout the day.

• Frequently uses a personal vehicle to travel within the service area.

• Standard office equipment operates frequently, including computers, phones, and other necessary devices.

• Ability to walk on uneven terrain and to ascend and descend stairs during home visits.

• Constantly speaks, communicates, interprets, or exchanges information accurately.

• Occasionally moves, transports, and positions objects weighing up to 50 pounds.

• May involve occasional exposure to environmental conditions in client homes, such as pets, smoke,

and odors.
