Occupational Therapist (OT)

Facility: Lake City Healthcare and Rehabilitation Position: Occupational Therapist (OT) Location: Lake City, Florida Schedule: PRN Pay Rate: $57 per hour Join Our Rehabilitation Team Lake City Healthcare and Rehabilitation is seeking a compassionate and dedicated Occupational Therapist to join our rehabilitation team on a PRN basis. In this role, you will help residents restore independence, improve functional abilities, and achieve their highest level of performance through personalized, evidence-based therapy interventions. Key Responsibilities Perform comprehensive occupational therapy evaluations and assessments. Develop individualized treatment plans based on each resident's goals and clinical needs. Provide evidence-based therapeutic interventions to improve activities of daily living (ADLs), mobility, strength, coordination, and cognitive function. Monitor resident progress and adjust treatment plans as appropriate. Educate residents, families, and caregivers on therapy techniques and home exercise programs. Maintain accurate, timely, and compliant clinical documentation. Collaborate with physicians, nursing staff, speech and physical therapists, and other interdisciplinary team members. Ensure compliance with all federal, state, facility, and professional standards. Foster a positive, respectful, and resident-centered environment that promotes dignity and independence. Qualifications Graduate of an accredited Occupational Therapy program. Current and active Florida Occupational Therapist license. Active Level 2 Background Screening (or ability to obtain). CPR certification preferred. Previous experience in a skilled nursing, rehabilitation, or long-term care setting is preferred.
