Full-Time | Skilled Nursing Facility
Salary: Up to $68,000 per year

Water’s Edge Rehab & Nursing Center at Port Jefferson is seeking a detail-oriented and personable Admissions Coordinator to join our team. This role is ideal for someone who thrives in a fast-paced healthcare environment and is passionate about providing a seamless admissions experience for residents and their families.

Position Overview

The Admissions Coordinator is responsible for managing the intake and admission process, serving as a key point of contact for referral sources, hospitals, families, and internal departments. This individual plays a vital role in ensuring efficient transitions into the facility.

Key Responsibilities

• Coordinate all aspects of the admissions process from initial referral through resident admission
• Communicate with hospitals, case managers, families, and referral sources
• Review clinical and financial documentation to ensure eligibility and appropriateness for admission
• Verify insurance coverage and obtain necessary authorizations
• Collaborate with interdisciplinary teams to facilitate smooth and timely admissions
• Maintain accurate records and census tracking
• Provide facility tours to prospective residents and families
• Assist in developing and maintaining strong relationships with referral sources
• Ensure compliance with all applicable regulations and facility policies

Qualifications

• Prior experience in admissions, intake, or healthcare coordination preferred
• Experience in a skilled nursing facility or healthcare setting strongly preferred
• Knowledge of insurance verification, including Medicare, Medicaid, and managed care
• Strong communication, customer service, and organizational skills
• Ability to multitask and work efficiently in a fast-paced environment
• Proficiency with electronic medical records (EMR) systems; PCC experience a plus
