Schedule: Day shift

Contract: 13 weeks

Guaranteed Hours: To be confirmed

Experience: SNF or post-acute experience preferred

About the Role

We are hiring a Physical Therapist for a contract opportunity in Lakeport, CA. This role is a great fit for a compassionate PT who enjoys helping patients improve mobility, restore function, and reach their highest level of independence in a skilled nursing and post-acute care setting.

Responsibilities

Evaluate and treat patients in skilled nursing and post-acute settings
Develop individualized treatment plans based on patient needs and goals
Provide therapy to improve mobility, strength, balance, function, and independence
Collaborate with interdisciplinary care teams for coordinated patient care
Document treatment, progress, and outcomes accurately and timely
Participate in care conferences and quality improvement efforts
Support a patient-centered and compassionate therapy environment

Requirements

Active California Physical Therapist license required
Skilled nursing or post-acute experience preferred
Strong clinical assessment and treatment planning skills
Excellent communication and documentation skills
Dependable, flexible, and team-oriented
Commitment to high-quality and compassionate patient care
