Pharmacist, PRN

Position Overview Pharmacist, PRN – the City of Hope pharmacy department seeks a dedicated professional to provide medication management, monitoring of drug therapy, and support for oncology and inpatient care. The pharmacist will work on a PRN (as needed) basis, covering all shifts and days in the 24‑hour inpatient pharmacy setting. Responsibilities Verify and assess medication orders to ensure alignment with patient‑specific factors (lab values, vitals, symptoms, disease states, and medication history). Optimize medication therapies using established clinical guidelines, ensuring safety and efficacy. Document all pharmacist interventions accurately and communicate with the healthcare team. Provide discharge education to patients and caregivers regarding medication regimens. Direct and supervise pharmacy technicians, ensuring adherence to protocols and guidelines for sterile intravenous compounding, chemotherapy, TPN, unit‑dose preparations, and medication refilling. Collaborate with interdisciplinary rounds, oncology teams, and medical staff, offering medication expertise and support for treatment plans and symptom management. Serve as a liaison between providers and pharmacy, addressing medication orders and resolving drug‑related issues promptly. Act as a resource for drug information, EHR drug order workflow, and assist clinical staff with medication queries. Compensate sterile compounded products in compliance with USP797 and USP800 guidelines. Participate in quality assurance and patient‑safety programs, contributing to departmental and hospital initiatives. Collaborate with the Pharmacy Clinical Coordinator to educate and assess pharmacy staff, providers, clinical staff, patients, caregivers, and pharmacy students. Qualifications Graduated from an ACPE‑accredited College of Pharmacy. Active license with the State Board of Pharmacy in a Doctor of Pharmacy or other advanced degree (highly preferred). Minimum of 2 years of institutional pharmacy experience, or strong interest in institutional and oncology pharmacy. Commitment to patient safety, quality care, and interdisciplinary collaboration.
