Job Opportunity: Bariatric Nurse Navigator
Our organization is looking for a dedicated Bariatric Nurse Navigator to assist patients on their weight-loss surgery journey in San Antonio, TX. This role for a Registered Nurse (RN) involves coordinating care before and after surgical procedures, performing assessments, delivering education on bariatric topics, and encouraging positive behavior changes.
Key Responsibilities- Coordinate pre-operative and post-operative care for patients.
- Conduct thorough assessments to evaluate patient needs.
- Provide comprehensive education regarding bariatric procedures and lifestyle changes.
- Support patients in making behavioral adjustments to enhance their health outcomes.
- Collaborate with surgeons, dietitians, behavioral health specialists, and primary care providers to create personalized care plans.
- Monitor patient progress and address any complications that may arise.
- Advocate for patients throughout their entire bariatric service experience.
- Utilize data tracking to drive quality improvement initiatives.
- Current RN license.
- Experience in bariatric nursing or a related field is preferred.
- Strong communication and interpersonal skills.
- Ability to provide culturally sensitive care.
- Commitment to patient advocacy and support.
