Registered Nurse (RN) - Home Health Case Manager - Southwest Suburbs

REGISTERED NURSE HOME HEALTH

LOCATION: Southwest suburbs with focus on Oak Lawn, Burbank, Justice, Bridgeview, Hickory Hills, Willow Springs, Chicago Ridge. HOURS: Days with weekend and holiday rotation. Must be able to work one summer and one winter holiday, and rotate on an on‑call schedule. This position is also eligible for up to a $10,000 sign‑on bonus subject to all taxes. You must be employed in the exact position at time of payout. Internal team members are not eligible for this sign‑on bonus at this time. The Home Health division of Advocate Health Care is the largest home‑health and hospice services in Illinois. The RN will provide home care services to the home‑bound population and establish strong relationships with patients and caregivers in order to assist the patient in reaching their maximum potential. Key responsibilities: Provide home‑based nursing care in patients' homes, ensuring continuity of care from hospital discharge. Assess, plan, implement, and evaluate nursing care for assigned patients. Set holistic goals that involve patients and families. Coordinate with the care team (social worker, CNA, MD, etc.) and document all care through point‑of‑care documentation. Develop and maintain a therapeutic nursing/patient relationship. Lead at the unit level and demonstrate mastery of clinical skills. Ensure quality care and uphold patients' right to receive high‑quality services.
