Responsibilities
- Provide direct nursing care to residents in a long-term care setting under the supervision of a registered nurse
- Administer prescribed medications and treatments according to policy and clinician orders
- Monitor vital signs, assess health changes, and promptly report to the supervising RN
- Assist residents with activities of daily living, promoting safety, dignity, and independence
- Document care provided accurately in the chart and ensure proper handoffs at shift change
- Collaborate with the interdisciplinary team and participate in care planning and routine quality tasks

Requirements
- Active LPN license in West Virginia
- Ability to work 12-hour shifts in a long-term care environment
- Strong communication and teamwork skills
- Eligibility to complete required credentialing and orientation processes
- Willingness to participate in the assigned weekend rotation and fixed shift schedule
