This is a full-time benefited position. Monday - Friday, 8-5.

Responsibilities:

• Plans, coordinates, implements, and evaluates the nursing care of all patients.
• Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs
• Works closely with the interdisciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.

Requirements:

• Must be at least 21 years of age.
• A valid non-restricted RN license in the state of Colorado is required.
• Must have a valid non-restricted driver's license.
• Flexibility and availability to work a weekend schedule if needed.
• At least 1 year of home health or hospice experience preferred.
