RN Case Manager

Good Samaritan Health Center seeks an RN Case Manager to coordinate holistic, patient-centered care for underserved individuals and families. In this role, you’ll conduct assessments, develop and manage care plans, support transitions of care, and connect patients to primary care, dental, behavioral health, and community resources. You’ll collaborate closely with an interdisciplinary team in a mission-driven, faith-inspired environment, promoting health equity and whole-person wellbeing. Ideal candidates bring strong clinical assessment skills, care coordination experience, and a passion for serving diverse, low-income communities.
