In this role, you will provide personal care, comfort, and emotional support to patients and families. Home Health experience is preferred, but a caring, proactive, and team-oriented mindset is just as important. You’ll be supported by an interdisciplinary team while enjoying the autonomy to structure your day and make a meaningful difference in the lives of patients and their loved ones. EDUCATION: Certified with OH state licensure. EXPERIENCE: We are looking for clinicians who are passionate about patient care and eager to make a meaningful impact. The ideal candidate will have strong communication skills, the ability to build trusting relationships with patients and families, and the professionalism to coordinate care with our team.
