Primary Care Physician - Montgomery, AL

Job Type: Full-Time, Mon - Fri (No Weekend, Nights, On-call)
Position Summary
\nWe are seeking a compassionate and patient-focused Primary Care Physician (PCP) to provide comprehensive care to a senior/geriatric patient population. The role offers a collaborative, team-based environment focused on coordinated care, quality outcomes, and an exceptional patient experience.
\nKey Responsibilities
\n· Provide comprehensive primary care to senior and geriatric patients.
\n· Evaluate, diagnose, and treat patients according to established standards of care.
\n· Manage a patient panel of approximately 500 or fewer patients.
\n· See approximately 8-13 patients per day.
\n· Participate in team huddles and collaborative care planning.
\n· Maintain continuity of care and a high-quality patient experience.
\n· Complete accurate and timely EMR documentation.
\n· Collaborate with multidisciplinary care teams to support patient outcomes.
\n· Review quality outcomes and medical records with leadership.
\nQualifications
\n· MD or DO from an accredited institution.
\n· Active, unrestricted Alabama medical license.
\n· Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
\n· Active DEA license.
\n· Medicare and Medicaid provider numbers.
