Inpatient Case Manager RN

Sinai Hospital's inpatient Care Management team provides discharge planning, transitional care coordination, complex care management, and utilization review through a multidisciplinary team of registered nurses, LPNs, licensed social workers, and administrative staff. While many functions are dictated by regulatory requirements, the core clinical focus is always on ensuring a safe and effective transition to the next appropriate level of care. Your work directly supports safe discharge planning, reduces barriers to care, and improves patient outcomes. About the Role: The Inpatient RN Case Manager at Sinai Hospital works with the clinical team and medical provider to coordinate and implement safe discharge plans for patients. Their main goals are to improve patient well-being, outcomes, and ensure healthcare services are used efficiently and timely. Key Responsibilities: Assessment & Planning: Conducts initial and ongoing assessments to determine patient needs for care coordination and discharge, then develops a focused discharge plan, especially for high-risk patients. Intervention & Collaboration: Works closely with the clinical team and medical providers to put the discharge plan into action. Continuous Improvement: Stays current with healthcare trends, regulations, and payer requirements related to patient care, discharge planning, and benefits. *This position offers a potential of up to $10,000 sign-on bonus. * Requirements: Basic professional knowledge; equivalent to a Bachelor's degree; working knowledge of theory and practice within a specialized field BSN preferred; ADN required 3-5 years related experience; Prior experience with inpatient case management/discharge planning required Registered Nurse License - Current Maryland license or eligibility to obtain Maryland license Case Mgmt cert preferred within 3-5 yrs of hire Pay Range: $40.12 to $62.19 Based On Experience Medical, dental, and vision coverage starting the first of the month after hire, plus wellness programs and mental health support 403(b) retirement plan with employer match, pension, and life and disability insurance Tuition reimbursement toward advanced degrees and specialty certifications Generous paid time off, fitness discounts, and free on-site parking About Sinai Hospital Sinai Hospital of Baltimore is a nationally recognized academic medical center, a designated Level II Trauma Center, and the largest community hospital in Maryland.
