As a Social Services Director - Geriatric Focus, you will play a vital role in ensuring the delivery of comprehensive social services to our residents. Reporting to the Executive Director, you will be responsible for developing, implementing, and evaluating programs and services that meet the unique needs of our geriatric population. Responsibilities Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the physical, emotional, and social needs of our residents Provide counseling, support, and resources to residents and their families to help them cope with the challenges of aging and long-term care Recruit, train, and supervise a diverse team of social workers, counselors, and support staff Develop and maintain relationships with community resources, organizations, and healthcare providers to ensure access to quality services and support for our residents Conduct assessments, identify needs, and develop plans to address the social, emotional, and spiritual needs of our residents Participate in resident care planning and coordination, ensuring that all aspects of care are considered and integrated into the overall plan Maintain accurate and detailed records, reports, and data related to resident care and services About the Role As a Social Services Director - Geriatric Focus, you will have the opportunity to make a meaningful difference in the lives of our seniors.
