Description
- Conduct thorough evaluations of patients in inpatient and outpatient settings and develop personalized treatment plans
- Implement physical therapy interventions targeted at enhancing mobility, strength, balance, and overall function
- Work closely with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
- Guide patients and their families through therapeutic goals, discharge strategies, and home exercise regimens
- Keep detailed records of evaluations, treatments, and progress in alignment with facility and regulatory standards
- Participate in care team rounds and interdisciplinary meetings to ensure cohesive patient care
Education
- Completion of a Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution
Certifications
- State license as a Physical Therapist
- CPR certification is preferred
- Specialized certifications (e.g., in geriatrics or neurology) are advantageous but not mandatory
Skills
- Significant experience preferred
- Ability to work with patients facing complex medical challenges
- Strong communication and collaboration skills are essential
