Flexible and personalized scheduling Great work-life balance Monday through Friday RN Case Manager Responsibilities The RN Case Manager's duties include, but are not limited to: Complete an initial assessment of patients and their families to determine hospice care needs. Regularly re-evaluate patients' nursing needs. Use assessment data to develop and update individualized care plans. Collaborate with the interdisciplinary healthcare team, patients, and families to ensure coordinated, continuous care. Counsel patients and families regarding end-of-life care and support. Initiate appropriate preventive and palliative nursing interventions. Communicate with physicians regarding patient needs and report changes in condition. Supervise Hospice Aides.
