This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.RESPONSIBILITIESUnderstands and adheres to established policies and proceduresProvides physician-prescribed physical therapyImproves or minimizes residual physical disabilities of the patient and returns the individual to an optimum, productive level within the patient's capabilitiesParticipates with all other home care personnel in patient care planningDirects and supervises personnel as requiredResponsible for initial assessment, plan of care, maintenance program development and modifications, and reassessments every 30 daysPerforms all skilled procedures as ordered by the physician; consults with physicians regarding changes in treatment and writes progress reportsInstructs patients and family/significant others in home programs and activities of daily livingParticipates in and presents inservice programs as assignedParticipates in Quality Assessment and Performance Improvement activities as assignedAttends all patient care conferences as scheduledPrepares clinical and progress notesAssists physicians with evaluating level of function and helps develop and revise the plan of careCompletes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition within Agency time framesAppropriately utilizes ICD-10 codesJOB RELATIONSHIPSupervised by the Director of Clinical Services/Clinical Manager.
