The position is onsite at our rehabilitation center, where you will work alongside a collaborative team of healthcare professionals. Key Responsibilities

• Conduct comprehensive evaluations of patients in a home care context, assessing their physical condition and mobility limitations arising from injury, disease, or surgery.
• Develop and implement personalized treatment plans that incorporate various therapeutic techniques, including exercise regimens, manual therapy, and the use of specialized equipment.
• Continuously track and document patient progress, adjusting treatment plans as needed to help patients reach their therapeutic goals.
• Educate patients, caregivers, and family members on therapy techniques and the proper use of assistive devices.
• Maintain accurate records while complying with state regulations and documenting all aspects of patient care and therapy interventions.
• Stay current with industry trends and advancements by attending workshops, engaging with professional literature, and participating in relevant organizations.

Patient Population / Client Focus

The physical therapist will work with a diverse patient population in various stages of recovery. This includes, but is not limited to, individuals recovering from orthopedic surgeries, rehabilitative needs following strokes, post-operative care, and chronic pain management. The role requires a compassionate approach, as many patients will need encouragement to regain their mobility and independence.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is required; a Doctorate of Physical Therapy (DPT) is preferred.
• Active state licensure to practice physical therapy in New Jersey.
• Minimum of two years of professional experience in a clinical setting, demonstrating a strong foundation in physical therapy techniques.
• Excellent communication skills, both written and verbal, necessary for patient interactions and documentation.
• Proficient in computer skills, with the ability to navigate electronic health records effectively.
• Current CPR certification is mandatory.

Preferred Experience

Compensation and Benefits
