Physical Therapist

The Physical Therapist will provide physical therapy service provision to adult Persons Receiving Services with Intellectual and Developmental Disability in the ITO setting on an Evaluative, Consultative, Restorative, Maintenance, and/or Habilitative level. They will be supervised by the Physical Therapy Supervisor 2.

Responsibilities:

• Collaborate with and serve as a member of the interdisciplinary team within the ITO setting; participate in meetings, present clinical information and observations, and provide consultation, training, and clinically relevant assistance to staff in other disciplines.
• Conduct Physical Therapy evaluations in accordance with OPWDD, New York State Department of Health, Centers for Medicaid and Medicare Services, and other third-party regulations.
• Develop, implement, and update Physical Therapy Plans of Care.
• Provide direct, individualized and group Physical Therapy services in accordance with the established Physical Therapy Plan of Care.
• Contribute to the interdisciplinary Comprehensive Functional Assessment.
• Maintain timely, accurate clinical documentation in compliance with OPWDD, New York State Department of Health, Centers for Medicaid and Medicare Services and other third-party regulations.
• Comply with all OPWDD, New York State Department of Health, Federal, State and Local regulations, policies, and procedures.
• Administer clinical supervision to Physical Therapist Assistants, as assigned.
• Monitor and procure supply inventories for physical therapy service provision and physical therapy related needs within the ITO.
• Maintain NYS Physical Therapy Licensure.
• Physical demands are considerable and include bending, lifting, standing, twisting and walking to assist Persons Receiving Service during physical therapy service provision and related trainings.
• Reimbursed travel. NYS driver's license is required.
• Additional responsibilities assigned by supervisor.
• Position will require adjusting your schedule to meet the needs of the ITO unit.

Minimum Qualifications:

Open Competitive:

• License and current registration as a Physical Therapist in New York State.

Transfer:

• Candidates must have one year of permanent competitive or 55-b/c status in the posted title and be eligible for lateral transfer; OR be eligible to transfer via Section 70.1, 70.4, or 52.6 of Civil Service Law. The transfer cannot be a second consecutive transfer resulting in an advancement of more than two salary grades.
