Provide ongoing support and advocacy for patients throughout their care journey. Maintain accurate documentation and reports of patient interactions. Participate in interdisciplinary care teams and contribute to care planning. Remain calm under stress and demonstrate patience, empathy, and compassion in all interactions. Qualifications Certification from an accredited social work program. Valid LMSW license. Excellent teamwork and collaboration skills. Strong interpersonal skills including patience, empathy, and compassion. Effective communication skills: active listening, clear writing, speaking, and reading comprehension. Ability to walk or stand for extended periods. Fast and adaptive problem‑solving abilities.
