We are seeking a Case Aide (Human Service Assistant 2) to join our Aging and People with Disabilities team in the North Bend office helping individuals and families in person .

Summary of Duties
As a Case Aide you will:
Work across all Aging and People with Disabilities program areas as a member of the service delivery team.
Assist case managers in providing long-term care services and financial benefits to older adults and people with disabilities.
Conduct regular home visits to support service delivery, including monitoring the effectiveness of service plans and assisting Oregonians with documentation required for long-term care services and financial benefits.
Support case managers by exercising judgment to recognize and assess clients' health and functional status and identify indicators of potential abuse or neglect.
Process and enter authorized payments for services such as homecare workers, in-home agencies, Meals on Wheels, Behavioral Support and Services, and Emergency Response Systems.
Communicate directly with service providers and Oregonians regarding long-term care services and benefits.
Coordinate and perform administrative and support activities, including documentation, data entry, transportation, client visitation, and communication with service providers.
Maintain accurate and timely case management records through computer data entry.

Minimum Qualifications
A valid driver’s license and acceptable driving record are required for this position
One year of experience assisting professionals in one or more of the following specialized social program areas:
Economically disadvantaged
Substance use and dependency
Disability Services
Child abuse and neglect
Juvenile delinquency
Any degree in a related mental health or human services program may substitute for six months of the required experience.
Essential Attributes
We are looking for candidates with:
Experience maintaining the confidentiality of sensitive information, including financial, medical, or protected health information (PHI).
Experience organizing and managing multiple priorities while adapting to frequent interruptions and changing workloads.
Experience collaborating with coworkers, clients, service providers, or community partners to coordinate services or accomplish shared goals.
Experience using and learning computer systems, such as Outlook and Excel, to accurately enter data, document information, maintain records, or process case-related information.

Working Conditions
The majority of your work will be performed in person at the ODHS office in North Bend , with regular travel to clients' homes, hospitals, nursing homes, and other community provider locations.
Your typical work schedule will be Monday through Friday, 8 a.m. to 5 p.m. Pacific Standard Time (PST). Occasional compensated overtime may be required to meet business needs.
Local travel is required. You may drive state vehicles to a variety of locations, including rural areas, in varying weather and road conditions.
You will interact with individuals and families from diverse backgrounds, including those who have experienced trauma and may be experiencing emotional or behavioral challenges.
You will work in a variety of home and community environments that may include unsanitary conditions, environmental hazards, animals, or exposure to contagious illnesses.

A valid driver’s license and acceptable driving record are required for this position.

The Oregon Department of Human Services does not offer visa sponsorship. Within three days of hire, you will be required to complete the US Department of Homeland Security’s I-9 form confirming authorization to work in the United States.
