Physical Therapist or PTA Home Health

Job Summary:

Homeside Rehab is seeking a compassionate and skilled Physical Therapist or PTA to join our pediatric home health team. In this role, you will deliver individualized, evidence-based therapy to pediatric patients in their homes, making a meaningful impact while enjoying schedule flexibility and a supportive team environment.

About the Role:

As a Physical Therapist or PTA, you will work closely with our multidisciplinary team to provide high-quality, patient-centered care to pediatric patients in Fort Myers, FL. You will evaluate patients, develop individualized treatment plans, and provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes. With a focus on pediatric patients, you will work closely with families to ensure a collaborative and effective treatment approach.

Responsibilities:

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals

• Provide evidence-based PT interventions to improve motor skills, functional independence, and developmental outcomes

• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements

• Collaborate with the clinical team to ensure coordinated and effective treatment

• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans

Job Qualifications:

• Active FL Physical Therapy License (PTA) — required

• Basic computer proficiency for electronic documentation

• Strong clinical assessment and interpersonal communication skills

• Ability to work independently in a home health environment

• Valid FL driver's license and reliable transportation for home visits

• Pediatric experience is preferred

• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings

Job Details:

• Job Type: Full-time, Part-time, Per Diem

• Location: Florida, 33913, 33058

• Salary: $65 - $80 per hour

• Schedule Flexibility: Choose your own hours, day and evening shifts available, Monday to Friday, with weekend availability as needed
