Registered Nurse (RN) - Director of Case Management

Director of Care Coordination

Schedule: Day Shift, Monday - Friday

Assignment Details: Temporary position

Requirements:

• BSN required; master's degree in nursing, Health Administration, or Business Administration preferred
• Case Management certification (e.g., CCM or ACM) preferred
• Minimum of five (5) years of experience in an acute healthcare setting required
• Minimum five (5) years of progressively responsible leadership/administrative experience in Case Management, Care Coordination, Utilization Management, or a closely related function required
• Advanced knowledge of case management, care coordination, utilization management, discharge planning, patient progression, and transitions of care in an acute-care environment
• BLS/BCLS certification required

Responsibilities:

• Lead interdisciplinary clinical and operational programs and staff
• Oversee performance improvement and support regulatory or accreditation readiness
• Ensure effective care coordination and efficient patient progression through the healthcare continuum
• Develop and implement care coordination strategies to improve patient outcomes
• Coordinate with healthcare providers to facilitate patient transitions of care
• Promote a collaborative work environment and foster staff development
