A-Line Staffing is hiring an RN Case Manager for a home health/field-based case management opportunity in Louisville, KY. This is a 13-week contract-to-hire role supporting high-risk member assessments, care planning, and care coordination across the assigned territory.

• Job Title : RN Case Manager
• Location : Louisville, KY 40272 (on-site/field-based)
• Schedule : Monday–Friday, 8:00 AM–5:00 PM
• On-call: Required (rotating weekends)
• Hours: 40 hours/week
• Openings: 2
• Territory covered: Southern Jefferson County and all of Bullitt County
• Mileage reimbursement : $0.43 per mile

What You’ll Do

• Perform comprehensive assessments of high-risk members/patients
• Collaborate with primary care providers to implement individualized care plans
• Identify, address, and continuously reassess cost-efficient and appropriate levels of care
• Coordinate care transitions and work closely with physicians, social workers, discharge planners, and other stakeholders
• Engage specialty resources as needed to achieve optimal outcomes
• Maintain detailed documentation of clinical, functional, and outcome measures
• Identify opportunities for health promotion and illness prevention
• Advocate for patients while maintaining privacy and confidentiality

Required Qualifications

• Active RN license (KY license preferred; compact license accepted)
• Associate’s or Bachelor’s degree in Nursing or related field
• Ability to analyze complex medical information and make sound clinical decisions
• Required home health skills/experience including wound care, IV therapy, drain care, G-tubes, and patient education
• HCHB (Homecare Homebase) experience required
