Description

The County of San Mateo's Family Health Services Department is seeking a Senior Community Program Specialist to join our team! Family Health Services (FHS) is a division within San Mateo County Health that serves primarily families and children working towards their goals of improved health and brighter futures. This position is in the California Children’s Services (CCS) Program. The Senior Community Program Specialist will play an integral role in supporting the development, design, and implementation of a new CalAIM program called Community Supports. This position will also support building a billing infrastructure into an Electronic Health System to create a more efficient and streamlined billing method and encounter reporting for the Enhanced Care Management and Community Supports program, while also assisting in the Claims process to ensure optimal and accurate reimbursement from the Payer. This position will be responsible for the Community Supports outreach and delivery and will also receive and process the CS Caregiver invoices on the back end to support the new program and a new process. This role will have analytic responsibilities involving quality improvement, establishing new efficient and innovative systems and processes, and supporting the overall establishment and expansion of CalAIM within California Children’s Services. The Family Health Services Senior Community Program Specialist is expected to advance the goals and priorities of California Children’s Services- Enhanced Care Management and Community Supports program through management of multiple projects and improvement efforts. The position will report to the Senior Community Program Supervisor and will work closely with the CCS Clinical Services Manager on multiple quality improvement projects.

The ideal candidate will have the following knowledge, skills and abilities:

  • Be self-directed, take initiative and be solutions driven, with strong attention to detail.

  • Have excellent written and verbal communication skills.

  • Have 2-3 years of experience in quality improvement work.

  • Be able to analyze, interpret data and foster data-driven decision making.

  • Be able to utilize modern computing systems (Microsoft applications, Adobe), web-based technologies and productivity software, especially SharePoint; proficiency with Visio and Power BI is a bonus.

  • Demonstrate the ability to develop strong, collaborative working relationships with organizations and individuals from diverse backgrounds.

  • Possess the capacity to manage projects while focusing on a variety of assignments simultaneously, organizing and reshuffling priorities while maintaining deadlines and due dates.

  • Possess knowledge of public health service delivery for children, youth, and families from diverse backgrounds and cultures.

  • Be able to organize and reshuffle priorities while maintaining deadlines and due dates.

  • Have experience with improvement systems.

The preferred candidate will possess a Master’s degree in Public Health or related field.

The current Full-Time, limited term Senior Community Program Specialist position.

Note: This is a limited term, at-will position. Limited Term Employees are assigned to assignments not to exceed three years. They receive medical and dental benefits, similar to regular employees, and a defined contribution retirement plan.

Examples Of Duties

Duties may include, but are not limited to, the following:

  • Direct and review the work of program staff.

  • Plan, develop and maintain plans for a program or a major function of a program.

  • Develop and implement planning and research methodology that serves to identify service needs and priorities including the development of survey instruments for use in data collection.

  • Collect and analyze data and develop a community profile on specific programs.

  • Identify and pool existing resources in the community.

  • Implement a contract management system for service providers assigned to ensure compliance with individual project goals and overall program goals.

  • Analyzes changes in regulations; evaluates the impact upon program operations and drafts policy and procedural changes as required.

  • Arrange and/or provide assistance or staff services to service providers, community groups and other special interest groups.

  • Coordinate and work with agencies and individuals in a specific community program to identify problems or facilitate program goals.

  • Organize and coordinate workshops, meetings, or sessions for individuals and groups in the community involved in specific programs.

  • Evaluate specific projects, project goals, program goals and client impact.

  • Coordinate specific components of a service-delivery system by identifying, researching and analyzing potential or available resources, by mobilizing resources through preparation of required documents and materials and by maintaining functional and supporting linkages among service providers.

  • Provide technical assistance and consultation to the related agencies, community groups and other interest groups.

  • Hold meetings, provides information, identify gaps in services and determine clients' needs.

  • Prepares a variety of correspondence, periodic and special reports, informational publications, program documentation, policies, procedures and other written materials.

  • Perform related duties as assigned.

Qualifications

Knowledge of:

  • Basic supervisory principles and practices.

  • Principles, practices, and administrative requirements of the program to which assigned.

  • Contemporary social, political and economic trends and problems related to community services.

  • Principles of social planning and program development related to community services.

  • Specialized technical knowledge related to assigned program area.

  • Monitoring and evaluating systems for community services programs.

  • Formulation and function of community and volunteer organizations.

  • Current social and community needs.

  • Principles and practices in community development and services planning.

  • Methods applied to the collection and evaluation of statistical data.

  • Principles of community organization.

  • Applicable federal, state and local laws, rules and regulations and County and program policies and guidelines.

  • Computer applications related to the work.

  • Office administrative practices and procedures, including records management and the operation of standard office equipment.

  • Prepare concise reports and recommendations.

Education and Experience:

Any combination of education and experience that would likely provide the required knowledge, skills and abilities is qualifying. A typical way to qualify is

  • Five years of experience in an organization providing social services, which has included responsibility for community service development such as planning, evaluating, monitoring or coordinating projects.
