Job Details

We are hiring a Physical Therapist (PT) for a Home Health opportunity covering the Bronx, NY. This is a strong role for a therapist seeking flexibility, autonomy, and meaningful one-on-one patient care in the field, while still having dependable support behind the scenes.

The PT will evaluate and treat patients in the home setting, helping improve mobility, function, and overall quality of life. This role is ideal for someone who enjoys independence, relationship-building, and a more flexible day-to-day structure than a traditional clinic environment.

What You’ll Do

Provide physical therapy evaluations and treatment in the home setting
Develop individualized care plans based on patient needs and goals
Support patients with mobility, strength, balance, and functional improvement
Educate patients and caregivers on exercises, safety, and ongoing care needs
Document visits and patient progress accurately and in a timely manner
Coordinate with physicians, nurses, and the broader care team to support continuity of care

What We’re Looking For

Active Physical Therapist license
Home health experience preferred, but not required depending on overall background
Strong assessment, treatment planning, and patient education skills
Comfortable working independently in a field-based role
Organized, dependable, and able to manage a flexible schedule
Strong communication and collaboration skills
