We provide exceptional serious illness care, hospice care and grief support for adults and children. The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices. Field CNA The field CNA is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The field CNA is responsible for observing the patient, reporting these observations and documenting observations and care performed. Key Responsibilities Provide compassionate care, emotional support, and companionship to patients and families Assist with personal care, mobility, and daily living activities Monitor, document, and report patient condition and changes to the RN Ensure patient safety and proper use of equipment Maintain a clean, safe, and comfortable home environment Follow care plans, policies, and participate in training and team activities Position requires outside travel to Orland Park, Tinley Park, Lemont & surrounding area Education/Experience High school diploma or GED. Meets the training requirements of Illinois. Current CNA certification. Requires a valid driver’s license and a dependable, properly insured vehicle that complies with state and organizational standards.
