Nurse Navigator

As a Nurse Navigator at Hartford Healthcare, you will guide patients and families through complex care journeys across our network. In this role, you will coordinate appointments, referrals, and follow-up care; provide clinical education; and remove barriers to accessing services. You will collaborate closely with physicians, nurses, and support teams to ensure timely, patient-centered care. Working within our culture of teamwork, compassion, and excellence, you will leverage advanced technology, contribute to quality initiatives, and support smooth transitions across inpatient, outpatient, and community settings.
