Clinical Pharmacist Ambulatory

SUMMARY:
Reports to Director Brown University Health Pharmacy, Director Specialty Pharmacy, or Ambulatory Manager as applicable. Under the supervision of the Pharmacist-In-Charge, Pharmacy Supervisor, Senior Clinical Pharmacist Specialist, or Clinical Coordinator is responsible for the interpretation, evaluation, preparation, and dispensing of medications pursuant to a legitimate order or prescription in keeping with established policies, procedures, standards of practice, and scope of pharmacy ambulatory services. Collaborates with multidisciplinary care providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost-effective medication use. Oversees and closely reviews the work of pharmacy technicians, pharmacy interns, and ancillary support personnel.

RESPONSIBILITIES:
Applies pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications for the patient populations served.
Ensures pharmacy services in area of practice are provided in accordance with established policies and procedures and federal and state rules and regulations.
Receives patient medication orders or prescriptions from practitioners in the outpatient setting. Reviews each order or prescription to determine therapeutic appropriateness of the patient's drug regimen as it relates to the patient's clinical status, concomitant drug therapy, and diet with consideration of the aging processes and human developmental stages (neonate, child, adolescent, adult, geriatric). Contacts the prescriber, or covering practitioner, as necessary to initiate appropriate interventions to maximize therapeutic efficacy, minimize potential drug therapy risks, and optimize cost effectiveness.
Provides optimal patient care by removing barriers to medication access. Fosters the growth of Brown University Health Pharmacy through patient recruitment.
Provides education and instruction on use of medications to patients and their families, as required.
Provides accurate and comprehensive information about drugs including drug interactions, potential side effects or adverse reactions, incompatibilities, and instructions for administration, to physicians, nurses, and other healthcare practitioners.
Participates in education and training of healthcare practitioners, physicians, nurses, pharmacy interns, pharmacy technicians, pharmacy students, and other care providers on matters relevant to medication use. May serve as a preceptor to pharmacy residents and interns.
Participates in continuous quality assessment and improvement activities and total quality management initiatives, including, but not limited to the adverse drug reaction, pharmacist clinical intervention, and medication incident reporting programs.
Maintains professional competence through participation in Pharmacy staff development and skills enhancement programs, ongoing independent study, education-related professional activities, and affiliations.
Performs immunizations or medication administrations in accordance with Rhode Island Board of Pharmacy Rules and Regulations, dependent on specific position and responsibilities.
Performs other duties as assigned.
iSpecific Retail and Specialty Pharmacy Responsibilities:
Reviews, approves, and enters medication orders in the computerized pharmacy information system including appropriate product formulation, dosage, frequency, and duration of therapy. Ensures relevant patient specific parameters are included on the patient's medication profile (i.e., diagnosis, known allergies, weight, age, etc.). Reviews and approves all medication orders and patient specific parameter computer entries if delegated to technician staff.
Dispenses ordered medications in the appropriate quantity to ensure patient receives the correct medication, in the right dosage and dose form, at the proper time.
Reviews and approves formulations for medications requiring compounding or manipulation prior to dispensing. Checks the final product prior to dispensing medication, if preparation is delegated to technician staff, in accordance with established procedures.
Maintains appropriate documentation and retains records regarding drug purchasing, storage, preparation, and distribution in accordance with established policies and procedures and existing federal and state rules and regulations.
iAdditional Specific Specialty Pharmacy Responsibilities:
Adheres to the policies and procedures of the Specialty Drug Patient Management Program (SDPMP) and encourages patient participation in the Program.
Performs patient outreach and assessments in accordance with the SDPMP.
Participates in the development and execution of patient Plans of Care.
iSpecific Medication Access Program (MAP) Responsibilities:
Provides MAP refill authorization services to Brown University Health patients per existing collaborative practice agreements and protocols in accordance with established policies and procedures and existing federal and state rules and regulations.
Navigates the patient's electronic health record (EHR) to investigate and answer questions related to MAP services. Completes and documents medication benefit investigations, prior authorizations, medication reconciliations, and/or medication financial assistance services. When appropriate, provides alternate therapeutic drug recommendations according to established guidelines.
Monitors accuracy and efficiency of MAP technicians refill processing, prior authorization submissions, denials and appeals management, and coordination of medication deliveries. Provides real-time feedback to improve case management and outcomes.
Participates in identifying operational trends and issues to help develop systems and workflows to improve productivity and efficiency and promote MAP success. Ensures MAP activities operate in accordance with established clinical standards of care and meet or exceed patient quality standards.
iSpecific Clinic-based Program Responsibilities:
Monitors and interprets laboratory results and adjusts medication regimens per existing protocol and collaborative practice agreements in accordance with established policies and procedures and existing federal and state rules and regulations.
Identifies out-of-scope patient care issues and triages them to appropriate member of the patient's healthcare team.
MINIMUM QUALIFICATIONS:
BASIC KNOWLEDGE:
Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of pharmacy.
Prior to start date, possesses current unrestricted licensure as a licensed pharmacist, or as a licensed 90-day temporary pharmacist, in the State of Rhode Island. Obtains full licensure prior to expiration of 90-day temporary pharmacist license, then possesses and maintains current unrestricted licensure as a licensed pharmacist in the State of Rhode Island.
Within 6 months of start date, for positions that perform immunizations or medication administrations in accordance with Rhode Island Board of Pharmacy Rules and Regulations, must possess and maintain an American Council for Pharmacy Education (ACPE) approved Immunization Certification and American Heart Association Basic Life Support (BLS) Certification
EXPERIENCE:
Two to three years progressively more responsible related experience, preferably gained in a similarly diverse and operationally complex health care environment is desired.
Knowledge of pharmacy information systems, EHRs, automated dispensing technology and personal computers.
Occasional travel between facilities or sites as required.
Effective interpersonal and communication skills.
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:
Extended periods of time spent standing and walking.
Requires the visual and manual dexterity to operate a computer.
INDEPENDENT ACTION:
Performs independently within department policies and practices.
Refers specific complex problems to supervisor where clarification of departmental policies and procedures may be required.
