LPN – Full Time D/E Rotation (Contract)

Location: New York 13165
Pay: $1,306 – $1,474/week
Schedule: Full Time, 5x8-hour shifts (Days/Evenings as needed)
Shift Options: Day and/or Evening rotation
Requirements: Every other weekend & holidays required
Note: Must be able to start on a Tuesday or Thursday for full-shift orientation

Position Summary

Provide direct and indirect nursing care under the supervision of a Registered Nurse
Deliver age-appropriate care to adult and geriatric residents
Support a safe, collaborative, and patient-centered care environment

Key Responsibilities

Collect and report resident physical and psychosocial data to RN and care team
Assist in developing, implementing, and updating individualized care plans
Monitor outcomes and recommend revisions or referrals as needed
Collaborate with interdisciplinary team, residents, and families
Maintain clinical competency through continuing education and training
Participate in staff mentoring and orientation as needed
Follow safety, infection control, and facility policies

Qualifications

Active New York LPN License (Required)
BLS Certification (AHA or RQI only – Required)
Minimum 1 year LPN experience in a Long-Term Care (LTC) setting
Experience with adult and geriatric patient populations
Ability to work in environments with potential exposure to blood/body fluids
