Position Overview

We are seeking an experienced Registered Nurse – Case Manager for a 13-week contract assignment in Carmichael, CA. This position supports hospital-wide Case Management/Care Coordination , with responsibilities covering both Utilization Management (UM) and Discharge Planning .

Key Responsibilities

• Manage approximately 15–20 patients/consults per day
• Complete initial case management assessments
• Perform Utilization Review (UR) and discharge planning
• Work with InterQual (IQ) criteria
• Coordinate patient care across all areas of the hospital
• Develop and facilitate effective discharge plans
• Work with multiple payers, uninsured patients, and homeless populations
• Collaborate with hospitalists, clinical teams, patients, families, and other stakeholders
• Utilize Cerner for documentation and care coordination
• Prioritize and reprioritize workload based on patient and department needs

Requirements

• Active RN License
• BLS Required
• Recent California Dignity Health experience REQUIRED
• Prior Case Management/Care Coordination experience
• Experience with Utilization Management and Discharge Planning
• Familiarity with InterQual criteria preferred — training will be provided
• Strong communication, collaboration, teamwork, and organizational skills
• Ability to manage changing priorities throughout the day

Schedule & Weekend Requirements

• 8x5 Day Shift
• Department hours are generally between 8:00 AM–5:00 PM
• Weekend coverage: Every other weekend depending on staffing
• Minimum expectation: at least one full weekend per month
• Holidays are not required, but candidates may work holidays if willing

Orientation

• 5 days of Care Coordination new employee orientation
• Additional unit orientation will vary based on the candidate's experience and needs

• Hospitalist/Intensivist: In-house
• Respiratory Therapy: 24/7
• Pharmacy: 24/7 availability not provided
• Coverage: All areas of the hospital
• No outpatient or home-care responsibilities

• MUST HAVE PRIOR RECENT CA DIGNITY HEALTH EXPERIENCE. Candidates without recent CA Dignity experience will NOT be considered.
• 2 supervisory references.
• Copy of Driver’s License required at submission to validate legal name.
• Candidates may not have been  directly employed (full-time, part-time, or PRN) by any facility associated with CommonSpirit, CHI, and/or Dignity Health within the past year .
• Candidates who were employed PRN through an outside staffing firm may be considered.
