The OB/GYN Triage serves as a primary clinical screener for patient calls, urgent walk-ins, and acute clinical inquiries within the OB/GYN practice. This position provides timely, safe, and clinically appropriate assessment of patient concerns; determines the appropriate level and urgency of care; and facilitates escalation to the appropriate provider or level of service.

The OB/GYN Triage utilizes established clinical protocols, algorithms, and evidence-based nursing practices to assess symptoms, identify potential emergencies, coordinate urgent care, and ensure effective communication between patients, providers, and the clinical care team.

This position requires compliance with the Center’s Compliance Program and Standards of Conduct and applicable policies and procedures. Such compliance will be considered as part of a regular performance appraisal. Failure to comply with the Center’s standards, including the failure to report any conduct or event that potentially violates legal or compliance requirements, will be met by the enforcement of disciplinary action, up to and including possible termination.

Essential Functions

• Perform telephone and in-person clinical triage for OB/GYN patients utilizing standardized protocols, algorithms, and established clinical guidelines.

• Assess patient symptoms, clinical history, and presenting concerns to determine the appropriate level and urgency of care.

• Identify signs and symptoms requiring immediate provider evaluation, emergency services, hospitalization, or other urgent intervention.
• Provide appropriate nursing guidance, education, and follow-up instructions within the scope of licensure and established protocols.
• Respond appropriately to urgent walk-ins and acute clinical inquiries while maintaining patient safety and timely access to care.
• Recognize clinical situations outside the scope of independent nursing assessment and promptly escalate concerns to the appropriate provider.

Provider Communication and Care Coordination

• Communicate clinical findings and patient concerns directly to OB/GYN physicians, nurse practitioners, physician assistants, and other members of the care team.
• Coordinate urgent provider appointments, emergency referrals, hospital admissions, and other necessary interventions.
• Facilitate communication between patients and providers regarding acute or time-sensitive clinical concerns.
• Coordinate with hospitals, emergency departments, specialists, and other external healthcare providers when necessary.
• Ensure appropriate documentation and follow-through of clinical recommendations and patient dispositions.

Documentation and Compliance

• Accurately document telephone encounters, triage assessments, clinical recommendations, provider communications, referrals, and dispositions in the electronic medical record.
• Maintain confidentiality and comply with HIPAA, organizational policies, nursing standards, and applicable federal and Virginia regulations.
• Follow established clinical protocols, standing orders, escalation procedures, and quality and patient-safety standards.
• Participate in quality improvement activities related to triage, patient access, clinical outcomes, and patient safety.

Additional Responsibilities

• Collaborate effectively with physicians, nurse practitioners, medical assistants, referral coordinators, front-office staff, and other members of the healthcare team.
• Assist in maintaining efficient patient flow and timely access to appropriate clinical services.
• Participate in staff meetings, education, competency assessments, and required organizational training.
• Perform other duties as assigned by the OB/GYN Director, OB/GYN Operations Manager, Clinical Supervisor, or designated leadership.

Required Qualifications

• Current, active Virginia Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing.
• Minimum of two (2) years of clinical experience in OB/GYN, Labor & Delivery (L&D), Women's Health, or a closely related specialty.
• Demonstrated clinical assessment and triage experience.
• Knowledge of OB/GYN clinical conditions, prenatal and postpartum concerns, gynecological emergencies, and appropriate escalation procedures.
• Ability to independently recognize urgent and emergent clinical situations and appropriately escalate care.
• Strong verbal, written, interpersonal, and organizational skills.
• Ability to communicate effectively with patients, providers, and multidisciplinary healthcare teams.
• Proficiency with electronic medical records and clinical documentation.
• Ability to prioritize multiple clinical inquiries while maintaining accuracy, professionalism, and patient safety.

Preferred Qualifications

• Bachelor of Science in Nursing (BSN).
• Experience in an OB/GYN outpatient practice, women's health center, or maternal-fetal medicine setting.
• Experience utilizing telephone triage protocols or nurse advice/triage systems.
• Current BLS certification.
• Experience with electronic health record systems.
• Experience coordinating emergency referrals, hospital admissions, and urgent provider evaluations.
