Nurse Practitioner – Home-Based Primary Care | Passaic County, NJ HarmonyCares is a leading national provider of value-based, in-home primary care for patients with complex healthcare needs. We are seeking a Nurse Practitioner to serve patients in Passaic County, NJ and the surrounding area . This role is ideal for an NP who wants to focus on patient care, not volume . Our providers typically see 6–10 patients per day , with approximately 30–60 minutes per visit , allowing time to manage complex and chronic conditions and build meaningful relationships with patients and families. What You Can Expect 30–60 minutes with each patient Collaborative, interdisciplinary team support Value-based care model focused on quality and outcomes Competitive base salary of $145,000–$160,000 plus annual bonus for average total annual earnings of $170,000 - $210,000 Comprehensive benefits, 401(k) match, CME/licensing stipend, and malpractice coverage with tail What You'll Do Provide comprehensive primary care to medically complex adult patients in their homes, including: Assessing, diagnosing, and treating acute and chronic conditions Managing patients with multiple comorbidities Ordering and interpreting diagnostic testing Prescribing and managing medications Educating patients and caregivers Collaborating with the interdisciplinary care team Qualifications Board certification Federal DEA license or eligibility Active CPR/BLS certification Valid driver's license and good driving record Primary care experience with adult patients Ability to travel locally throughout the assigned territory Experience in geriatrics, home-based care, home health, hospice, palliative care, or complex-care populations is preferred. #J-18808-Ljbffr
